 根據國際糖尿病聯盟公佈的數據顯示2007年全球約有2.46億人口罹患糖尿病預估在2025年罹患糖尿病的人口數將會增加至3.8億。糖尿病在國人十大死因中排名第四,由於發生率有逐年攀升之勢,對國人健康的威脅也日益增加。近年來研究發現「代謝症候群」會大大增加罹患糖尿病的風險,罹病率是正常人的930倍。在歐美先進國家與台灣,代謝症候群的盛行率分別是20~30%17~23%。代謝症候群是集合多重危險因子,它包括了幾項代謝因子的異常:葡萄糖不耐症、胰島素阻抗、中央性肥胖、高血脂症及高血壓;這些代謝因子異常也同時是增加罹患心血管疾病(增加24)的風危因子。過去幾年本研究室在此領域已經累積了一些相關經驗,我們利用高膽固醇-果糖飲食誘導實驗大白鼠產生代謝症候群。在此動物模式中我們觀察到並非每一個組織都會同時產生胰島素阻抗,胰島素阻抗是選擇性的發生於部分組織中如:紅色骨骼肌、肝臟與心肌;由於此動物沒有產生肥胖症狀,我們也觀察到其內臟白色脂肪組織並未發生胰島素阻抗。我們亦發現此實驗動物不僅有心肌胰島素阻抗的現象,同時亦併發有擴張性心肌病變與心臟收縮功能不全。為什麼並非所有的組織都有胰島素阻抗而是選擇性地發生在某幾個特定的組織、以及組織發生胰島素阻抗的先後順序與血液生化質與代謝小分子的濃度變化間之關係都有待進一步研究釐清;另實驗動物在尚未進展成糖尿病之前即已經產生嚴重的心肌病變與收縮功能不全,其病理機制仍有待進一步實驗釐清。除此之外、我們也發現紅酒中的一個多酚類小分子白藜蘆醇 (resveratrol) 對第一型與第二型糖尿病皆具有明顯的降血糖與降血脂的效果,我們發現白藜蘆醇具有類胰島素之作用包括:促進骨骼肌與脂肪攝取葡萄糖、促進肝醣合成、改善葡萄糖不耐症與增加胰臟分泌胰島素(第一型糖尿病);此研究成果在長庚大學育成中心的協助下已經順利的申請到台灣與美國的專利。接續下來我們也利用高膽固醇-果糖的胰島素阻抗動物模式評估白藜蘆醇是否可改善代謝症候群與其藥理作用機轉。研究結果發現白藜蘆醇能有效地改善全身胰島素阻抗,降低血中胰島素、膽固醇、三酸甘油脂與游離脂肪酸,同時亦有顯著的降血壓作用。我們發現白藜蘆醇改善骨骼肌胰島素阻抗是透過活化雌激素受體(ER-alpha);且我們發現RSV促進骨骼肌攝取葡萄糖的作用在早期(1小時)是透過Erk/p38、而晚期(14小時)是透過p38/PI3K/Akt的訊號傳遞路徑。
